Support for a Safe and Comfortable Start

Transitional Services

Dexiros offers transitional support services for individuals receiving BI, CAC, CADI, and DD waivers who are moving into their own home in the community. These services help individuals establish a safe and functional living environment by providing assistance with moving, furniture assembly, light cleaning, essential household item shopping, and coordination of setup.

For the purposes of this service, a person’s own home is defined as a residence they own, rent, or lease that is not operated or controlled by a service provider.

To begin services with Dexiros, case managers must complete the online Referral Form. Please ensure that Dexiros is clearly listed as the authorized provider for transitional services (T2038) in the client’s CSSP. Once the referral is received, we will coordinate directly with the client and case manager to schedule services. Please allow 7–10 business days for planning and scheduling, depending on service complexity.

Eligibility

A person is eligible to receive transitional services if they meet all the following criteria:

  • Age 18 or older
  • Enrolled on the Brain Injury (BI), Community Access for Disability Inclusion (CADI), Community Alternative Care (CAC) or Developmental Disabilities (DD) waiver, or reasonably expected to enroll within 180 days or enrolled in the Moving Home Minnesota (MHM) program
  • Moving from an eligible setting to their own home or an integrated community supports setting
  • Moving to a setting where these items and expenses are not normally furnished
  • Not able to access transitional services from other funding sources (e.g., community nonprofit organizations).

Eligible Settings

A person can use transitional services to move to their own home or an integrated community supports setting from any of the following eligible settings:

  • Adult foster care home (corporate and family)
  • Assisted living facility that provides customized living services
  • Hospital
  • Institution for mental diseases (IMD)
  • Intensive rehabilitation treatment or rule 36 setting
  • Intermediate care facility for persons with developmental disabilities (ICF/DD)
  • Nursing facility
  • Unlicensed setting (e.g., person’s own home, family home or integrated community supports setting).

Dexiros provides a wide range of covered services to support a smooth and stable transition into a new home. From essential furniture and household items to moving costs, utility setups, and even pest treatments, we make sure individuals have the foundational support they need to live safely and independently.
